If you don’t have insurance please keep in mind we turn no one away based on their ability to pay. Albany Wellness Clinic offers eligibility assistance at our primary care location.

This service includes:

  • Assistance with understanding their Medicare benefits for 64 year olds who are approaching Medicare age;
  • Assistance in completing paperwork for Medicaid, LaCHIP and Expanded Medicaid
  • Certified Application Counselors assist patients with Market Place options
  • Sliding Fee scale application for those at or below 200% of the Federal Poverty Level

What is Medicare?

Medicare is the federal health insurance program for:

Medicaid is a joint federal and state program that, together with the Children’s Health Insurance Program (CHIP), provides health coverage to over 72.5 million Americans, including children, pregnant women, parents, seniors, and individuals with disabilities. Medicaid is the single largest source of health coverage in the United States.

  • People who are 65 or older
  • Certain younger people with disabilities
  • People with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD)

What is Medicaid?

Medicaid provides health coverage to millions of Americans, including eligible low-income adults, children, pregnant women, elderly adults and people with disabilities. Medicaid is administered by states, according to federal requirements. The program is funded jointly by states and the federal government. (Medicaid.gov)

States establish and administer their own Medicaid programs and determine the type, amount, duration, and scope of services within broad federal guidelines. Federal law requires states to provide certain mandatory benefits and allows states the choice of covering other optional benefits.

Benefits of Medicaid Health Insurance coverage for specific services:

Mandatory benefits include services including inpatient and outpatient hospital services, physician services, laboratory and x-ray services, and home health services, among others.
